Funeral Benefit

Document Requirements

• Completed GSIS Funeral Benefit Application Form
• Death Certificate of the deceased GSIS member (PSA or Certified Local Copy)
• Marriage Contract (PSA copy) if surviving spouse is claimant
• Official Receipt of funeral expenses if claimant is not the spouse
• Valid ID of the claimant

Application Process

1. 1Download and fill out the GSIS Application Form for Funeral Benefit.
2. 2Obtain a certified photocopy of the Death Certificate.
3. 3Submit application via GSIS Wireless Kiosk (GW@PS), GSIS Touch app, or email to the regional branch.
4. 4Submit official receipt of funeral expenses if the claimant is not the surviving spouse.
5. 5Wait for credit to the claimant's eCard or bank account.
