Application Process
- 1Ensure active PhilHealth membership prior to delivery.
- 2Submit copy of Member Data Record (MDR) and valid ID to the billing section of the hospital.
- 3Fill out PhilHealth Claim Form 1 (CF-1) and Claim Form 2 (CF-2) at the hospital.
- 4The Newborn Care Package is automatically deducted from the hospital billing for both mother and child.
