Completed Sickness Benefit Application online
Medical Certificate (Form MM-1) signed by attending physician
At least 3 monthly SSS contributions in the 12-month period immediately preceding the semester of sickness
Laboratory/diagnostic results supporting the diagnosis
Enrolled bank account (DAEM)

Application Process

1If employed: Notify your employer within 5 days of sickness onset. The employer will submit the claim to SSS.
2If self-employed/voluntary: File the SSS Sickness Benefit Application online via My.SSS portal within 14 days of sickness onset.
3Submit medical certificates and laboratory results for review by the SSS Medical Evaluation Department.
4Receive daily cash reimbursement in your enrolled bank account (or via your employer).
